charts updated to 2012d

Some minor edits to the wild plays, and I’ve also made a couple of cosmetic changes:

1. added page numbers so that a play index/glossary can be created
2. gave names to the “roll 1/roll 2” results on the injury chart (now called “minor” and “extended”)

I’m hoping #2 will make the new injury charts a bit easier to follow, and I’ll try to get a post up soon with some example injury resolutions using the new labels.
